Why you break out on your back and chest

Short answer

Two different conditions look almost identical on the back and chest. True body acne responds to benzoyl peroxide and salicylic acid. Fungal acne, which is malassezia in the follicle, does not respond to either and is why people treat for months with no result. If the bumps are small, uniform and itchy, and worse after sweating, treat it as fungal with an antifungal wash.

The clearest tell is uniformity plus itch. Acne is untidy: different sizes, different stages, blackheads mixed in. Fungal acne looks like someone stamped the same small bump forty times.

A course of oral antibiotics that improved your skin briefly and then made it worse is a strong clue for fungal. Antibiotics reduce the bacteria that keep malassezia in check.

Treating fungal acne

The treatment is a dandruff shampoo used as a body wash, which sounds improvised and is genuinely the standard approach.

  1. Use a ketoconazole 1% or zinc pyrithione shampoo on the affected area in the shower.
  2. Leave it on the skin for three to five minutes before rinsing. This is the step people skip, and it is the step that matters.
  3. Daily for two to four weeks, then two or three times a week to keep it away.
  4. Stop applying oils and heavy occlusive body products to the area. Malassezia feeds on certain lipids, and coconut oil in particular tends to make it worse.
  5. Shower promptly after sweating rather than sitting in damp clothing.

Improvement usually shows within two to three weeks, which is faster than acne treatment and is itself a useful confirmation that you diagnosed it correctly. It recurs, because malassezia lives on everyone's skin permanently, so keep the maintenance washes going through summer and after workouts.

Treating true body acne

  • Benzoyl peroxide 4 to 10% wash. Higher strengths are more tolerable on the body than the face. Leave it on for a minute or two, and be aware it bleaches towels and bedding.
  • Salicylic acid body wash or a leave-on spray for the areas you cannot reach.
  • A body spray with glycolic or salicylic acid for maintenance between washes.
  • An adapalene gel on the worst patches if washes alone are not enough.

The friction and occlusion side matters as much as the products. Sports bras, backpacks, weight benches, tight synthetic tops and long car journeys all press bacteria and sweat into follicles. Sitting in gym clothes after training is the most common single contributor, and showering within about twenty minutes changes more than most products do.

Long hair with conditioner and styling product running down the back at the end of a shower is another common cause, and the fix is simply to wash your body after you rinse your hair rather than before.

Scarring, marks, and when to get help

Body acne marks fade more slowly than facial ones because skin there is thicker and turns over more slowly. Expect months, not weeks, and use sunscreen on shoulders and chest through summer because sun makes the marks considerably darker and more permanent.

True scarring, meaning indented or raised tissue rather than a flat dark mark, does not resolve with any product. Keloids on the chest and shoulders are common in people prone to them and are worth treating early.

See a doctor if it is painful and deep, if it is scarring, or if it has not improved after three months of consistent treatment of the correct type. Body acne responds well to oral treatment and there is no reason to spend two years on washes.
