Dark marks that last for months after a breakout

Short answer

Flat brown or black marks left after a breakout are post inflammatory hyperpigmentation, or PIH. They are pigment, not scars, and they do fade. Treat the acne first, then use azelaic acid, tranexamic acid, alpha arbutin or a gentle retinoid, and wear sunscreen daily. Expect eight to twelve weeks minimum, and three to six months for older marks.

Is it a mark or a scar?

This decides everything, and people use the word scar for both.

  • Flat and discoloured, brown, black or purple, level with the surrounding skin. That is pigment. It will fade, and the products below work.
  • Indented or raised, a dip or a bump you can feel. That is textural scarring. Topicals will not fix it. That needs procedures such as microneedling, lasers or subcision from a professional.

Run a finger over it with your eyes closed. If you cannot feel it, it is pigment and you are in the right place.

Treat the acne first

This is the step people skip and it is the reason progress stalls. Every new breakout creates a new mark, usually faster than your serum is fading the old ones. You cannot win that race.

If you are still breaking out, sort that first or in parallel. Adapalene, benzoyl peroxide and azelaic acid all have strong evidence. Azelaic acid is the efficient choice here because it treats the breakout and the mark at the same time.

Picking is the other half of this. A spot you leave alone leaves a mark for weeks. A spot you squeeze leaves one for months. A hydrocolloid patch over it costs about twenty five cents and mostly works by stopping you touching it.

Add one at a time, roughly a week apart. Stacking four actives in week one is how people end up with a wrecked barrier and more marks than they started with.

Why gentler works faster on deeper skin

This is the most important thing on this page and it is counterintuitive.
